We serve patients in Northern Utah through two offices. On Monday, Tuesday, and Wednesday we are in Logan at: 150 E 200 N Suite F, Logan, UT 84321. On Thursday we are in South Jordan at: 3648 W 9800 S South Jordan, UT 84095. Our offices are closed Friday – Sunday and on all public holidays.
